List the last known holder, issue date, document reference and whether a complete scan exists. If a folder was lost during courier delivery, include the dispatch receipt and tracking history without assuming that every document inside has become legally invalid.

The next question is whether the information remains with its issuer. A registered birth may still be evidenced through the registry even when the family's paper certificate is missing. By contrast, a private agreement may require obtaining another party's signed counterpart. If no underlying record can be found, the task may involve a separate legal procedure rather than ordinary reissuance. Do not describe all these routes as getting a duplicate; that phrase can conceal an important difference in provenance.

Protect identity information while preserving evidence of loss

Where passports, signatures, health information or a child's identifying details were inside the missing bundle, assess the exposure as well as the inconvenience. Write down when the loss was discovered, who last had access and whether unauthorised use is suspected. Contact the relevant issuer through an independently verified channel for document-specific reporting instructions. A family should not assume that losing a birth certificate triggers the same cancellation procedure as losing a passport.

Use the issuer's repeated-certificate route

The SDA's current repeated-certificate page identifies the categories entitled to receive civil-registration certificates, including parents and specified guardianship categories for birth certificates, and a notarised authority route for another person. It also identifies application and identity-document requirements and service channels. Having paid a surrogacy agency does not, by itself, establish that an applicant falls within an entitled category. Where legal parentage or representative status is disputed, resolve the authority question before arranging collection.

Provide the information needed to locate the actual registered record and ask which supporting authority documents are required in the individual case. Check whether the proposed online channel is available to the applicant and the relevant document type. Confirm current charges and collection arrangements directly. Published processing options for a straightforward repeat certificate do not guarantee that a complex identity or registration problem will be resolved within that period. Define the representative's task narrowly

An overseas parent may need someone in Georgia to request, collect or forward the replacement. These are separate actions and should be described in the authority document where needed. Identify the record, the competent institution and whether the representative may obtain copies, receive a refusal or arrange onward certification. Do not assume that a broad agency service contract will be accepted as a notarised power of attorney for a public-body request.

Before executing a foreign authority document, obtain the recipient's current form and authentication requirements. Make a practical plan for showing the applicant's identity without sending original passports through unnecessary intermediaries. Record who may receive the finished document and require a delivery receipt. The representative's administrative mandate does not make that person a parent or guardian. If the previous agency held authority and a new representative is appointed, separately assess revocation and notification so that two people do not act on inconsistent instructions.

Recover private agreements through their actual custodians

For a lost private agreement, identify every signer and ask whether a signed counterpart or authenticated electronic version remains. Request the complete set, including schedules, amendments, language versions and signature pages. An invoice or a chat saying that a contract was signed may help trace the transaction, but it is not necessarily the same evidential object. Mark an unsigned draft accurately; do not add signatures from another file to make it appear complete.

Where the arrangement is legally a mandate, Civil Code Articles 713–715 provide relevant rules on information, confidentiality and return of what was received or obtained. Their application depends on the actual relationship and documents. A demand for the file should identify particular items and the basis for requesting them, rather than claiming ownership of every record held by the agency. Keep medical and family-record recovery separate

A clinical record may contain information about several people with different rights. The person financing services is not automatically entitled to another adult's entire health file. Identify whose data are needed, why they are needed and who can lawfully request them. Ask the institution about its patient or representative access procedure. If the immediate task concerns evidence for a child's application, specify the relevant document instead of demanding a complete clinical archive.

An agency's translated summary should not be relabelled as a hospital-issued original. Ask the issuer whether it can provide an authorised copy, a replacement document or confirmation of an existing record. Preserve distinctions between a contemporaneous entry and a later explanation. Do not ask a clinician to backdate a new statement or recreate a missing event from memory as though it were the original record. Any remaining evidential gap should be disclosed to the professional reviewing the application.

Build a provenance note for every replacement

Record the original document reference if known, the replacement's issue date, the issuer, the applicant and the reason a replacement was requested. Explain whether the new document reproduces an existing entry or records a later correction. A different certificate number or issue date may be consistent with a proper replacement, but the recipient needs enough context to understand the difference. Do not conceal those changes to make the file look identical to the lost set.

Retain the request, receipt and delivery evidence alongside the replacement. A short index can link the lost item, surviving scan and replacement without making unsupported claims that every page is legally equivalent. Where a recipient questions authenticity, refer it to the issuer's verification method rather than offering personal assurances. If an agency supplies a file, ask whether it obtained it directly from the issuer or from another intermediary. Each additional step should remain visible in the evidence trail.

Rebuild only the downstream steps that are actually needed

A surviving scan of an old apostille should not be pasted onto a newly issued certificate. Identify exactly which document and signature were authenticated, and ask the competent authority how the new document should be prepared for its destination. A replacement might require a fresh authentication step or another accepted verification route. The answer depends on the document, the receiving institution and the accepted form, not simply on whether the previous bundle had a stamp.

Check translation requirements again after replacement. Issue dates, reference numbers and correction annotations may have changed even when the underlying birth facts have not. Reusing an old translation without comparison can create a fresh discrepancy. Keep certified pages together and record their order before courier dispatch. Tell pending recipients what changed

Make a list of applications in which the lost set has already been used. Separate an application not yet filed, a preliminary review, a formally pending matter and a completed decision. Ask each current recipient whether it needs the replacement, an explanation of loss or another specific item. Use the existing application reference and avoid submitting a second application without instructions. A replacement birth certificate may not affect a completed decision in the same way that replacement of an identity document does.

For a Chinese first passport or travel-document application, the May 2026 Embassy checklist identifies several evidence categories and allows further requests. A new birth certificate does not replace parents' passports or evidence of their residence or visas at the child's birth. Explain the provenance of the replacement accurately and ask how it should be added. Hypothetical example: the courier loses an authenticated set

Suppose a courier loses a Georgian birth certificate, its translation and authentication pages after a family has submitted scans to a recipient in China. A complete digital copy remains. The family should preserve the dispatch evidence, confirm precisely what was enclosed and contact the recipient about the pending file. It should then use the competent issuer's replacement route, subject to authority checks, and determine the certification and translation needed for the replacement.

The new certificate may bear a later issue date. The covering note can explain that it was issued from the existing registration after loss of the paper bundle, if that is what the issuer confirms. The old scan remains historical evidence of what was submitted; it should not be presented as the physical replacement. This hypothetical example does not guarantee acceptance, nationality or travel. When no source can be located, document the unsuccessful searches

Ask each relevant custodian for a clear response identifying the search performed and the reason it cannot supply the requested item. Was the wrong institution contacted, is the record under a different spelling, is authority insufficient, or does the record not exist? These explanations lead to different next steps. A statement that an agency cannot find its copy does not establish that the registry or clinic has lost the underlying record.

Prepare a chronology of requests and responses before considering a procedure to establish a legally significant fact or challenge a refusal. The correct route depends on the fact, the evidence and the authority's competence. Do not treat witness statements as an automatic substitute for every formal document, or ask private parties to create official-looking replacements. If a deadline is approaching, ask the recipient about a reasoned extension or supplementation option while preserving the available evidence. Availability of that option must be confirmed, not assumed.

Finish with a checked handover and a useful backup plan

When the new set arrives, inspect names, dates, pages, seals and attachments before forwarding it. Record who holds the physical originals, where a restricted digital copy is stored and which institutions received which version. Use a delivery method appropriate to the sensitivity of the documents, and obtain confirmation of receipt. Avoid keeping the only usable copy in an agency employee's personal account. A family should be able to locate its essential records without relying on one intermediary.
